Select a form by double-clicking on its title. If you have “Adobe Acrobat Reader” installed on your computer the file will automatically open. From the new window, you can print it or save it to your hard drive by ‘right-clicking’ on the file name and selecting “Save Target As”. If you do not have “Adobe Acrobat Reader” installed you can download from Adobe’s website by clicking here. The program is free. After installing the program restart your browser and return to this page.
